This page provides data on work-related fatalities for cases in which the employer was issued a citation by Federal OSHA where at least one violation was related to the fatality. These incident summaries include only cases in which citations and penalties were issued for fatalities that occurred on or after January 1, 2017. OSHA investigates all work-related fatalities in all covered workplaces. OSHA has up to six months to complete an investigation and determine whether citations will be issued. Employers must report worker fatalities to OSHA within eight hours.
